New Students

  1. Admission (SPS Office): Secure the list of requirements for admission. Fill-out the Entrance Examinee Data Form (UPHSL/DJGTMU-SPS-EED-02) and sign the consent as required by the Data Privacy Act (UPHSL/DJGTMU-SPS-CDP-87).
  2. Cashier: Pay for the entrance examination/locator number fee.
  3. Admission (SPS Office): Present the official receipt to be issued a permit to take the entrance examination. Claim the examination result/certificate of passing (refer to the QPR-QMS-11, 5.1.9 – Entrance Examination Procedure). Present admission requirements for evaluation of the admission officer. Issuance of payment slip for enrollment kit.
  4. Cashier: Pay for and claim the enrollment kit.
  5. Admission (SPS Office): Fill out the Individual Inventory Form (UPHSL/DJGTMU-SPS-IIF-14A) and Student Needs Assessment Survey (UPHSL/DJGTMU-SPS-SNA-36). Encode student admission data for subsequent printing (refer to QPR-QMS-11, 5.1.16). When necessary, an undertaking for incomplete credentials should be executed (please refer to the QPR-QMS-11, 5.1.17). Issuance of Drug Test Monitoring Slip for notation at the Clinic.
  6. Clinic: Sign notice for the random mandatory drug testing. The authorized personnel of the Clinic (e.g. physician, nurse) will sign the drug test monitoring slip as proof that the student accomplished the form.
  7. Dean’s Office: Fill out pre-enrollment form for issuance of assessed subjects upon submission of Dean’s envelope initialed by SPS personnel.
  8. Cashier: Pay for the assessed tuition and other fees. Secure the official receipt.
  9. Registrar’s Office: Present official receipt with assessed courses at the Registrar’s Office for issuance of official registration form.
  10. ITS Office: Present Registration Form along with the accomplished drug test monitoring slip to the ITS office for issuance of the student identification card (ID).


New Students

(Maritime only)

    • Ishihara (Color Blindness Test)
  5. Dean’s Office: Present results of Medical Examination and will be interviewed by the Dean or the designated staff.
  6. Note: Applicants who failed the Audiometry and Ishihara tests shall be advised to take other offered programs in the University. He/She have the option to validate the test results with other laboratory facilities.
  7. Admission (SPS Office): Present admission requirements together with medical exam result. Issuance of payment slip for enrollment kit.
  8. Cashier: Pay for and claim the enrolment kit.
  9. Admission (SPS Office): Fill out the Individual Form and Student Needs Survey. Encode student admission data for subsequent printing (refer to QPR-QMS-11, 5.1.16). Transferees

Old Students (Maritime Only)

  2. UPH Medical Center: Conduct of:
    • Laboratory Tests (CBC, Urinalysis, Fecalysis, Chest X-ray)
    • Audiometry/Hearing Test
    • Visual Acuity Test
  3. Dean’s Office: Submit results of Medical Examination and present accomplished semestral clearance (UPHSL/DJGTMU-SPS-SC-41A). Secure pre-enrollment form for advising and assessment of courses to be enrolled
  4. Registrar’s Office: Present official receipt with assessed subjects at the Registrar’s Office for issuance of official registration form.

 
 

Foreign Students
